=== 2nd generation cephalosporins === Early second generation cephalosporins are very similar in basic structure to the first generation. Loracarbef however does not have the normal dihydrothiazine ring but is a carbacephem that has a carbon atom in the ring instead of a sulfur atom making it a tetrahydropyridine ring. This chemical property gives loracarbef better stability in plasma while retaining oral absorption characteristics and affinity for binding to PBP. The 7-phenyl-glycine makes it orally available and the chlorine at position C-3 makes it as active as cefaclor. An important structural change in the development of second generation cephalosporins was the introduction of an α-iminomethoxy group to the C-7 side chain. This gave an increased resistance to β-lactamases due to stereochemical blocking of the beta-lactam ring. Cefuroxime was the first cephalosporin to incorporate this side chain. Another very important group in the second generation is the aminothiazole ring to the C-3 side chain. This development drastically increased binding affinity to PBP and increased antimicrobial activity. The aminothiazole ring can be seen in the structure of cefotiam.

==== Don Cossacks in World War II ==== In April 1936, the earlier ban on Cossacks serving in the Red Army was lifted. Later in 1936, two existing Red Army cavalry divisions were re-designated as Don Cossacks. In 1939, a number of these regiments were issued with traditional Cossack uniforms, in ceremonial and field service versions. The dress of the Don Cossack units included dark-blue breeches with broad red stripes which had distinguished them prior to the Revolution. The Don Cossack Cavalry Corps saw extensive active service until 1943, after which its role diminished, as did that of the other remaining horse-mounted units in the Red Army. However Don Cossack cavalry was still in existence in 1945 and participated in the Victory Parade in Moscow. During World War II, the Don Cossacks mustered the largest single concentration of Cossacks within the German Army, the XVth SS Cossack Cavalry Corps, a great part of them former Soviet citizens. The XVth SS Cossack Cavalry Corps included the 1st Cossack Division and the 2nd Cossack Division. The majority of the Cossacks remained loyal to the Red Army. In the earliest battles, particularly the encirclement of Belostok, Cossack units such as the 94th Beloglisnky, 152nd Rostovsky and 48th Belorechensky regiments fought to their death. In the opening phase of the war, during the German advance towards Moscow, Cossacks were extensively used for raids behind enemy lines.

The cognitive complications likely result from the antihistaminic effects of cinnarizine, while the motor effects are a product of the antidopaminergic properties. In cases of overdose, the patient should be brought to and observed in a hospital for potential neurological complications.

== Medical uses == The compound is indicated for adults with type 2 diabetes as an adjunct to diet and exercise to improve glycemic control. Dulaglutide is not indicated in the treatment of subjects with type 1 diabetes or patients with diabetic ketoacidosis because these problems are the result of the islet cells being unable to produce insulin and one of the actions of dulaglutide is to stimulate functioning islet cells to produce more insulin. Dulaglutide can be used either stand-alone or in combination with other medicines for type 2 diabetes, in particular metformin, sulfonylureas, thiazolidinediones, and insulin taken concomitantly with meals. The medication's phase 3 clinical trial program demonstrated reductions in hemoglobin A1c of approximately 1% with the 0.75 mg and 1.5 mg doses of the medication, along with approximately 5 pounds of weight loss on average. The higher 3.0 mg and 4.5 mg doses that were approved in 2020 demonstrated hemoglobin A1c reductions closer to 1.5% and slightly more weight loss. An alternate study conducted by REWIND had enrolled a lower-risk population consisting of 9901 patients, two-thirds had no prior cardiovascular events. These patients were randomized to dulaglutide 1.5mg weekly versus a placebo group for a median of 5.4 years. As a result of this study, Dulaglutide had reduced MACE by 12% (HR 0.88, 95% Cl 0.79-0.99) and had produced a 15% relative risk reduction in the composite renal outcome.
